How To Claim Your Acuvue Rebates
Please complete the following steps below to receive your rebate:
Have An Eye Exam: Take an eye exam and buy your supply of Acuvue lenses. Please note, this purchase must be made within 3 months (90 days) after the actual eye exam. Also keep in mind, it's ONE rebate per customer!
Enclose Box Tops & Eye Exam Receipt: Attach two box tops and your eye exam receipt together with the lens purchase receipt and send to: 2011 ACUVUE National Rebate 386-174 P.O. Box 410294 Dept C El Paso, TX 88541-0294
That's it! Just be advised the Acuvue rebates offer expires December 31, 2011 so get your claim in before this date. Allow approximately 6 - 8 weeks for your rebate but your part is done. Easy enough!
